Zariah is one of the kindest and most down-to-earth individuals I have interacted with during my time at UC.
I had the pleasure of working with Zariah this month. I am a third-year internal medicine resident and have had the opportunity to work with a lot of different social workers over the past 3 years. Zariah is one of the kindest and most down-to-earth individuals I have interacted with during my time at UC. She cares about her patients and always goes the extra mile to ensure they are actively involved in their care and received the best care possible. She always reaches out to me and other residents to run through patients' plans so she remains updated about their day-to-day changes.
In addition, she always gets things done. Whether it's figuring out if someone can go home next week on inotropes out of the CVICU or setting up at-home services and so many more. She gets it all done efficiently and faster and better than any of the other social workers/case managers I have worked with, and I think I have worked with almost every single one in the ICU and most on the medical services. We need more like her.

I am a resident physician in the Internal Medicine Dept at UC. I have had the pleasure of working with Ms. Zariah Swartz during my current rotation in the CVICU and I just wanted to pass along some positive feedback on my experience working with her. Zariah has been friendly, kind, helpful, flexible, and prompt to respond to all of my messages and inquiries. She helped me out with a very complex discharge the other day and went above and beyond to ensure that my patient had all of his many discharges and follow-up needs arranged for in a timely manner. He needed to discharge with dobutamine and Zariah was a great advocate on my patient's behalf to endure that the home healthcare company discharged my patient the same day he was medically ready. I have had that complex medical need (dobutamine) delay patients' discharge by a day or two (even longer), but Zariah was instrumental in getting it done for him promptly. Zariah has been an excellent communicator, has been flexible with the medical team when we need to adjust the timing of the social work rounds, and has been an upbeat and fun person to work with in general. I just wanted to pass my feedback along because Zariah has truly gone above and beyond to help residents and patients out.

I am one of the NPs with Cardiac Surgery that works closely with Zariah Swartz. I just wanted to let you know how much I and the cardiac surgery team appreciate everything that Zariah does for us and our patient population. Through the Covid pandemic and the stress it has placed on every worker to do the best for our patients, especially once they are ready to be discharged from our facility to either home or another facility, Zariah has made the transition as smooth as possible for them. The patient and family members always have concerns about sending a loved one to another facility, especially in lieu of the pandemic, and Zariah reassures them in a calming manner. She is very thorough when it comes to assessing our patients' needs and goes above and beyond to make sure those needs are met. She is on top of her work at the beginning of the workday, planning for future discharges and following up with any loose ends that my team or the patient may have.
For example, we recently had one patient who was cleared for home by PT/OT but with limited support. She was able to find a respite center that would accept the patient to help provide additional support for both the patient and family members. I am aware of the influx of various team members for every service at UCMC, even with the Case Managers/Social Workers. Zariah has shown that she can do her workload and that of others while consistently providing our teams and our patients with all the necessary needs to make a smooth transition from hospital to home or facility. I am speaking for myself, but I do believe that the rest of the Cardiac Nurse Practitioners and Cardiac Surgeons would agree as well.
