Ann is an Oncology Case Manager. She goes above and beyond for her patients. She is not afraid to talk about prognosis and care for patients and families. She makes sure that patients have resources such as nutrition, palliative care, and social work. There was one Friday that she was working from home, and one of our older patients called because she was out of colostomy supplies. Colostomy supplies are difficult to get after patients run out. The patient told her that their Home Health care agency told her about a place that has extra supplies and will give them to patients at no charge. Ann got the address of the company that had the colostomy supplies and got in her car, picked them up, and took them to her house. The patient was very grateful.
