She is diligent and her follow up on cases is excellent.
Carolyn joined the Care Management department 2 years ago. Carolyn has brought tremendous positive energy to the Care Management department. In addition to Carolyn carrying the house beeper to address Care Management issues, she is also responsible for a daily evening report of follow up items needing attention after 4p. Somehow, Carolyn gets it all done and manages it all with a positive attitude and a smile!
Carolyn is not only hard working and dedicated, she has also proven herself to be creative in her critical thinking skills and has a tremendous, positive impact on the patient experience. She is extremely patient experience centered. There have been several cases where Carolyn has gone above and beyond to help a patient/patient family.
As an on-call supervisor in the evenings, I have worked with Carolyn on two cases where the daytime care manager left for the day believing the patient would be a discharge the next day.
Both cases included extensive DME ordering and transportation authorization (which can take up to one hour on the phone). Often such cases require numerous calls and coordination with after hour services. Without hesitation, Carolyn dug right in to make it possible for the patient to go home with their family the same evening. The gratitude from the patient/patient family was overwhelming.
This nomination is not only being made due to the numerous patients compliments we have received but due to the extremely positive feedback from her fellow Care Managers. Often the Care Managers will have a difficult, time-consuming case they worked on all day yet are still waiting for medical clearance, an authorization, or additional information to complete the patient discharge. They can count on Carolyn to come through for them if the discharge happens after normal business hours. She is diligent and her follow up on cases is excellent. We often hear from Care Managers how they came in for their shift only to be pleasantly surprised that a CMA was completed and they can get an earlier start on their day. They are truly grateful to have Carolyn as a colleague.
