Maia consistently demonstrates excellence through her clinical expertise while providing compassionate care for her patients and families. I have observed Maria in the field; her expertise shines through, evident as each patient is served with her undivided attention. One situation stood out while visiting a patient recently discharged from the hospital who did not shy aware from sharing his anger with everyone. He was quick to compliment Maria by stating, ‘she saved my life by sending me to the hospital.’ His anger/frustration for his obvious decline, requiring more assistance from his live-in and wife, still came through. Maria was able to redirect him and provided excellent teaching, including the caregivers, throughout the visit. Unfortunately, the patient came home with peripheral edema, now weeping, with no supplies in the home. Maria reached out to the MD, obtained orders, reported her assessment, and, of course, had what the patient needed in her car. The patient was missing many prescribed medications; Maria confirmed and compared medications with MD and provided written instructions for the patient, spouse, and caregiver. Maria continued to provide support and care for this patient and his family until the patient eventually transitioned to our hospice team. Maria’s patients rarely call into the office as Maria is always in contact with them until the safe discharge is completed.
