In Jordan’s role, she calls patients one to two days after discharge, to review their status, medications, consults, and home health needs, and coordinates their care. Recently, she called a patient who was sent home from the hospital with a prescription for Eliquis after a diagnosis of Pulmonary embolism. When the patient went to the pharmacy to pick up the Eliquis, they were unable to afford the prescription. By the time Jordan was speaking with the patient, it had been over 24 hours since they had taken any anticoagulation medication. Knowing the risk of rehospitalization and death, Jordan contacted the inpatient provider, the patient’s primary care physician, social work, and pharmacy to develop a plan. Unable to figure out a plan, Jordan started thinking of other ways she could help this patient with their medication. Jordan found a way to get the medication for free for 30 days. This allowed the patient time to go to their primary care provider to figure out a long-term plan. She worked tirelessly to make sure this patient was safe at home. Thanks to Jordan, the patient did not have to be readmitted and started on anticoagulation therapy. Thank you, Jordan!
