It is Suzanne’s clinical knowledge that guides and educates our team of nurses to advocate and encourage patients and families with their chronic and or terminal illnesses.
Suzanne Platt brought her many years of experience and expertise as a Certified Palliative Care Nurse to our rapidly growing Hospice and Palliative Care Services in November 2019. With Suzanne’s expertise and guidance, we can accomplish the goal of Palliative Care, which include few readmits to the hospital and an overall positive patient experience during this challenging time in their lives. It is Suzanne’s clinical knowledge that guides and educates our team of nurses to advocate and encourage patients and families with their chronic and or terminal illnesses. Her continued support and dedication to the palliative care team is a true representation of Memorial Care Hospice and Palliative Care. Suzanne’s relentless engagement with over a hundred PC patients and their Physicians has resulted in countless transfers from Palliative care to Hospice.
Suzanne is an Extraordinary Nurse, and we are blessed to have her as part of our MemorialCare team. Suzanne is a compassionate nurse who not only has endless nursing knowledge but cares for patients in a subtle holistic approach. She is excellent at educating patients about a disease process, helping patients understand treatment options, and supporting patients’ educated choices without judgment. She works all hours of the day to ensure patients are comfortable and have a hospice transition when appropriate with grace and ease.
Suzanne not only cares for patients in her role as the Palliative Care Nurse Navigator, but she also cares for the Palliative Care team. She is always available to answer social workers' and nurses’ questions, call physicians to advocate for patients, and assist in any way possible to help achieve excellent care to allow the Palliative Care team to achieve success. She is generous with sharing her knowledge with the team and is careful with her words ensuring she does not come across as condescending. Suzanne serves as a bridge between hospitals, physicians, other MemorialCare Programs, DME companies, and the list goes on. Suzanne is a kind, competent, compassionate nurse that has dedicated her life to nursing, We at MCHPS cannot think of anyone who is more deserving of the DAISY Award!
