At the end of all of her calls, she had made contact with 43 different facilities. Due to her persistence, she found a homeless-to-home shelter in New Orleans that would take him.
We had a homeless patient who was wheelchair bound and was to be discharged. He had been unable to walk for some time but did not have anything he needed to be discharged safely. We attempted to get him into a rehab or skilled nursing facility, but he did not meet criteria. Mrs. Annette called every shelter in our region and no one would take him. At the end of all of her calls, she had made contact with 43 different facilities. Due to her persistence, she found a homeless-to-home shelter in New Orleans that would take him. Next, she needed to figure out how to get him what he needed to be discharged. She talked with a doctor about the issues. This doctor purchased the patient a WC. Annette went to Walmart and purchased a couple of outfits for the patient as he did not have any clothes with him. Annette spent more time than a lot of people would have to find this patient a place to stay and make sure he had what he needed upon discharge from the ED. This is only one small example of the compassion and empathy Annette has for her patients.
