Jenny Ho is our dedicated RN Case Manager covering the Neuro-Spine service. She recently coordinated a discharge for a complex and sensitive case and worked tirelessly with the family, providers, insurance administrators, and outpatient staff to create an extensive home, and hospital setting for the patient. Jenny worked with the outpatient infusion center to have weekly infusions done at the home until the patient was strong enough to return to the clinic. She worked with the insurance company to cover the transportation to vital outpatient appointments that the patient would need to monitor their progress. Most importantly, she worked extensively with the patient and family to make sure they had the education needed to safely discharge home. This safe and complex discharge took weeks to coordinate and could not have been completed without the dedication and devotion shown by Jenny Ho. The patient and family expressed their gratitude for the care and support they received from case management while at UCSF. Jenny truly went above and beyond. She is a role model for the nurse case manager profession.
