A patient on 5 Frederick was being sent back to the nursing home. Sadly, this patient's sister was receiving hospice care and was predicted to pass away at any moment. In order to spend one final minute with the patient, Jean managed to get in touch with EMS and arrange for her to be sent to her sister's home. Jean made extraordinary efforts to make this happen. When Jean informed the patient that she would have one final opportunity to visit her sister, the patient broke down in tears. I'm incredibly proud to work with Jean. More than any other case manager I've met, she fights for the rights of her patients. I've seen her in action, and she doesn't back down when it comes to standing up for her patients. We are tremendously fortunate to have Jean as our case manager here at 5 Frederick. Jean, we are so happy to call you our own. Never stop being who you are! We love you!
