Elena is the charge nurse for telemetry, but she does so much more than just keep the unit going. She is a very active and engaged participant in the readmission workgroup, specifically in the After Visit Summary work and the Congestive Heart Failure work. She has developed educational material for patients and staff. She is diligent in auditing ABSs and asks questions regarding why certain information is not available on the AVS. She has helped the group understand how completing the PTA med list and how subsequent ordering of meds affects the AVS.

Lastly, her unit recently had a patient who was frequently readmitted. She notified ancillary staff, including case management, our outpatient FQHC, hospitalists, and the chaplain of his admission, wanting to reduce his anxiety and encourage him to not leave AMA, as was his habit, until his medical issues could be stabilized. She frequently goes above and beyond in her role to improve the care of her patients and her staff.
