Leonila Estorque is a senior nurse and senior preceptor in C1 Pediatric Hematology, Oncology. Her flexibility to work as a bedside nurse and team leader is admirable. She has been one of the nurses committed to bedside care with the highest regard for patient safety. Her consistent vigilance as a staff in meticulously keeping track of her patient’s order entries to ensure that medications will be given in the right doses, as evidenced by two High Risk ‘Good Catch’ Medication prescriptions that were prescribed 10 times the actual patient dose. These two are among the significant High Risk ‘Good Catch’ identified which has saved patient lives and prevented potential patient harm as follows:
1. A physician mistakenly ordered 60 mg of hydralazine instead of 6 mg
2. Another physician also mistakenly ordered oral chemotherapy 750 mg/day instead of 75 mg/day.
