Through her quick actions & deep-rooted relationships across the nephrology network, Pat has protected many patients and extended many lives. 
Since 1997 Patricia Hall has dedicated her head heart and hands to patient care.  She tirelessly serves a community with clinical excellence behind all that she does.  Her ICHD center is always top 20% in the village. The two facilities she covers have three total modalities.  She supports the only HHD program and RN in the region and always raises her hand to partner and train PTs from nearby markets.  Pat has a legacy of best in class calibration on what she expects from her teammates; and she always builds intentional plans to elevate her team’s performance.  She is always patient-centered, has phenomenal relationships with her physician partners, and is ALWAYS available for her team.  

During late 2022 she was visited by the CMS for a recertification survey – the surveyor stayed for 3 full days.  Pat’s well-calibrated team filled with a highly talented CC was able to take this experience head-on. The survey came back with only one opportunity, notably a structural add.  At the end of the survey, the surveyor pulled both DCS and MCS aside to highlight what was evident during her time in the facility.  Pat and her team care DEEPLY about WHAT they do and WHY they do it.  She noted the amount of effort and care each teammate provides was top-notch.  The facility dynamic that Pat serves is unlike most; it involves an academic partner and some restrictions to external resources – Pat has never focused on this; has throughout her time build partners with this academic partner in EVERY lane, has a phone number for every teammate in each department and works through her relationships to get her patients what they need.  

One time, Pat had a patient who was unable to treat on site, due to having Covid.  Pat worked closely with the patient and their family to ensure the patient made it to the hospital, due to his elevated & concerning lab reports.  Proactively, she called the hospital and secured timely placement for the patient as he was in critical need.  She worked through difficult family dynamics to ensure the patient's family had what they needed to act and act quickly.  The patient's life was saved due to this fast action… but Pat did not stop here – she worked closely with her team to ensure the family had everything they needed during his stay at the hospital and upon returning to the facility.  She offered education and follow-up for the patient’s family and supported them in education including a Renal diet review.  Through her quick actions & deep-rooted relationships across the nephrology network, Pat has protected many patients and extended many lives. 

 For reference a patient that started when she did (1997) was treated at her unit for years; and is STILL on dialysis & enjoying his life as a working man.  In the last 6 months, Pat has had 3 separate power outages, one planned and two unplanned due to inclement weather.  She worked with her team to ensure EVERY single patient was treated in a timely manner if not same day the day after, she personally traveled to another facility to ensure the level of care provided off site was not compromised.  

When we try to celebrate Pat, she is always deflective; giving credit to her entire team, and consistently cites “I just did my job.” Her modest humility is admirable given how so many see her as an inspiration and how regardless of what is happening around her, her commitment to her patients NEVER waivers.  She is a usual first call for many as challenging patient scenarios come up as she has experienced so much and has a wealth of knowledge.  Pat is an ICON for our industry and has truly dedicated her entire self to serving others.  She has been with DaVita for 25 years supporting two++ facilities at many times throughout her tenure!
