Melissa’s relentless pursuit of patient advocacy & outcomes has been evident throughout a recent system pilot project aimed at reducing hospital days among the top tier of high-utilizing patients. Through her persistent collaboration with many key stakeholders across the care continuum: hospital, dialysis, ambulatory, community & county providers, patients involved in this pilot are able to increase quality days at home. Melissa has orchestrated many care conferences on behalf of the complex patients she is supporting, resulting in outcomes such as reduced ED and hospital utilization, decreased opioid utilization, and increased communication across the care continuum. She continuously applies critical thinking and honors the Fairview Commitments of seeking perfection, collaborating for outcomes, and achieving results. There is no doubt many of the patients Melissa has supported throughout this project and in her other roles have been able to live a higher quality of life as a result of her advocacy.
