Vanessa is an RN who works PRN for the Eastside Pediatric Clinic. Vanessa has been a compassionate team member and most recently demonstrated this by reaching out to children with complex healthcare needs in order to coordinate their care.

We have a requirement to reach out to patients who are on a list for the Colorado Regional Accountability Entity which manages patients who have Medicaid. Usually one of the Registered Nurse Care Coordinators (RNCC) would do this work. But we have been without an RNCC for several months now. I asked Vanessa if she would be willing to learn this complicated workflow and help us with our list of patients. Vanessa kindly volunteered to do so. She spent time learning about this workflow and dove into reaching these patients. She was able to improve our metric beyond the 75% goal to 90%. Vanessa took the time to provide care coordination to many of the patients that were on this list. Through her time and attention, patients were able to obtain the services that were necessary.
