Ines is a discharge facilitator in the AMS CSU. We had a patient who was admitted and diagnosed with a disease that meant he was not going to recover and that he would die within a few weeks. Sadly, this person had no family in the UK and was living in accommodation that would not be suitable to meet his needs or be suitable to provide the specialised care he would need as he approached the end of his life. Ines explored his extended family to see if anyone could support him but sadly they too were no longer alive. He had nowhere suitable to go and no one to look after him for the short time that he had left. Ines worked extremely hard to contact many external agencies that might be able to help him, such as the British Red Cross, going the extra mile to get hold of the right people, to ensure that all the best options for him to achieve a dignified death were explored. Ines escalated the patient to her head of nursing with concern that we could not let him be discharged to die alone as the staff on the ward were the only people he knew who cared for him. Ines also showed great compassion to the patient to make him feel safe and cared for in his last weeks of life so that he knew we would not let harm come to him and he could die in peace. Ines really showed dedication and compassion to this patient, which is at the heart of being a nurse.
