It is with great pleasure that I write to you today to nominate MELODY LEEDS, RN, for the prestigious DAISY Award.  I believe that Melody is the best choice for the DAISY award because she always goes above and beyond to help her patients and their families.

I can attest to that as I experienced that firsthand when I developed severe abdominal pain.

Melody happened to be a neighbor, and my apartment was flooded, requiring the maintenance man to come and remove all the water from the floors.  He noticed that I did not look good and was in severe pain. 

When he briefly went outside, he saw Melody pulling up to her parking spot. He approached her and told her that I did not look good and was in severe pain.  Melody did not hesitate and came to my apartment to assess me.  She told me that she would take me to the ER right now because the doctor needed to examine me and find out what was causing the severe abdominal pain.

Even though Melody had been working the whole day, she did not hesitate to bring me to the ER right away.  While in the ER, Melody was in contact with all the ER staff, assisting in any way she could.  Melody truly was my patient advocate and guardian angel! I couldn’t have done it without her!! (No immediate family for me in Florida)

Melody is a very professional, knowledgeable, and AMAZING nurse who really cares about her patients.  She saved my life by taking me to the Emergency Room!
Melody even stayed with me in the ER until 1 am in the morning when I (finally) received a room for admission.  

I am so fortunate that Melody was there to help me, as she helps ALL of her patients and their families!
