Jessica skillfully helped bridge the communication between the primary team, the palliative care team, and the mother and father.
Palliative Care was consulted for a patient and family that needed support and ongoing goals of care discussions. Jessica was one of the nurses who became the patient's primary nurse, and the patient was the mother and father's first baby. There were complicated comorbidities, and Jessica journeyed with the mother and father, helping them to understand what was going on with their baby. She attended all the family meetings even when she was not on duty to support and advocate for the patient, and mother and father who loved their child and wanted to make the best decision for their baby. Jessica skillfully helped bridge the communication between the primary team, the palliative care team, and the mother and father. When the mother and father decided to transition to comfort care for their baby, Jessica made sure that mother and father had all the legacy and memory-making accessories, prints, and photos of their baby. Jessica took gentle, loving care of the patient, making sure that she was clean and that her hair was combed with a ribbon, normalizing the situation. The mother felt reassured that her baby was well taken care of by Jessica. Jessica provided excellent, patient-centered, and family-centered care.
