We had a patient who was brought in with a mental health diagnosis that was Yvonne's patient. She made a connection with this patient and called the patient after she was discharged to make sure she had her follow-up appointments. This patient was not a local and it was important that this patient get into her appointments as an important part of her follow-up.

What really stood out to me was during the “Hospital Discharge Instructions," where so much info was being presented, Yvonne explained item to item she explained all those items that I could not understand OR that I questioned. Yvonne did not repeat what she told me, she rephrased the item/concept into words that I could understand and follow - ALSO she made use of the "schedule white board" drawing images/words/work flow . . . explaining that which I was unsure. From her notes ( and mine ) I entered my "safe" home confident that I could successfully negotiate my home's healthy recovery. When I had a question, I referred to those Discharge notes. In closing, Yvonne met my ride, and together with an assisting nurse, guided me out of my room to the curb and into my friend's vehicle. Her positive parting comments - you're going to be fine, YET if anything comes up, CALL US IMMEDIATELY, these words were spoken as I entered my friend's vehicle unassisted.
