Wube has demonstrated exemplary work throughout his DNP journey. It has been a pleasure to work with him and see his student commitment through his timely submission and completeness of assignments which speaks to his desire to offer his patients the highest level of care. As a Psychiatric Nurse Practitioner, Wube shows immense respect and compassion for those he serves. His DNP project addressed the need for coordinated care among his patients with severe mental health illness who struggled to schedule and attend outpatient services such as lab draws for medication adjustments and primary care visits. Additionally, Wube evaluated the number of patients with chronic disorders and excessive use of ED services. Recognizing a need, he created a coordinated program to schedule and assist patients with transportation to and from their appointments.

The results demonstrated a 98% show rate for appointments and labs, improved coordination of care among outside providers, and a 100% decrease in ED visits over the time his DNP project was implemented. Using resources within his practice, the budget was minimal as was staff time. Therefore, the project is sustainable with the opportunity to speak to the DNP essentials of policy change within organizations, expansion to other domains, and a decrease in overall chronic health problems and societal economic burden.
