Chelsea Kelleher is a full time Case Manager for VNA Health Group South Team C63. She has worked for our organization since 2018 when she came to us with lots of home care experience from Meridian at home (lucky us)! Chelsea has so many strong characteristics and if I could I would have checked all of the boxes above. She goes above and beyond every day to make sure her patients have all the correct information to remain knowledgeable and safe at home. Chelsea is one of those nurses who gets several compliments every couple of weeks. Her patients call asking to speak with me and talk about how they love her and don’t know what they will do once home care services end. Chelsea often gives and gives, always aiming to please her patients, her team, and her manager. Over these 5 years of working with her we have discussed many challenging cases, she always comes up with the perfect plan and makes sure the patient receives nothing but stellar care. Once she had a patient who she visited for close to a year and had 13 wounds from a rare skin disease. She worked closely with our wound support team and the wound care center where the patient was treated. To say this patient was challenging is an understatement as Mrs. M had a strong, demanding personally and was a smoke. Imagine doing wound care for 13 wounds, with the smell of smoke and wearing a mask during the time period of high risk Covid-19. Just one more tidbit about Chelsea - She is a mother of 3 small school-aged children and rarely misses a day of work. Her notes are always on time, she returns all emails/phone calls promptly, and even lets the scheduler know she can help take more visits. I just love Chelsea Kelleher and would highly recommend her to receive the 2023 DAISY Award!
