Andrea started as our High-Risk Patient Nurse about 8 years ago. She would call patients who had been in the hospital or were high risk for re-admission. She always went the extra mile to help patients- contacting social work or their pharmacies directly to close care gaps or ensure the patients had the help they needed. As providers, we recognized the superb medical care our patients were getting due to her tenacity and charisma. Of course, when you have someone outstanding, you never get to keep them for long, and she soon was promoted to spearhead our primary care medicare wellness visits and oversaw a team of nurses that were sent out to our offices to help. While COVID threw a wrench in the nursing staff available to do these visits, Andrea stayed with the MWV Team and helped grow our visits exponentially by streamlining the process in CareConnect and providing education to providers. Her greatness was once again recognized. She was promoted to lead our PACT team. I was so excited because everything Andrea takes ownership of, she does it to perfection, she doesn't stop until it is right. The PACT Team has been an incredible asset to us in primary care in just the few short months that it has been in place. Andrea is available and asks for feedback and leads a tremendous team of nurses. All of her work accomplishments aside, Andrea is always such a joy. She is incredibly approachable and a joy to be around. I wish we could clone her, but she is truly one of a kind. Thank you, Andrea, for all of your tireless efforts - they not only improve the patients' lives directly but also the lives of the providers caring for the patients.

Andrea Green is an impactful leader who provides quality, compassionate care to not only our patients but also her staff and co-workers. Andrea goes above and beyond, dedicated to our organization to support whatever is needed to help provide the best care to our patients. There is never a task outside of Andrea's responsibility or expertise. Right now, we have been going live with roll-outs of PACT that Andrea envisioned, created, and implemented while she also manages a Vaccine Call Center at the same time. She wears so many hats without dropping the ball and it is quite impressive in the way she handles the stressors of each transition with a high level of professionalism and optimism. She keeps strong communication, shares stats and data to support our needs and progress, and keeps all stakeholders in the loop on changes, which is hard to do with the volume of practices she supports. As a high leader, she knows the ins and outs of the tasks her staff completes and does not hesitate to roll up her sleeves and help because metrics/strong customer service and support are so important to her. As a peer to Andrea, I have so much trust in the care, management, and support that she gives. I am proud to work by her side and to have her here in our organization.
