Kayla is the chronic care nurse for our practice and wears many hats. She is not only a nurse to our patients, but a social worker, scheduler, counselor, and friend. There is not just one reason, but many, why I believe Kayla meets the criteria for a DAISY Award recipient. Kayla is well-respected by her patients. peers, and providers. Her passion for caring for our patients is evident in every conversation. Whether spending hours on the phone with insurance companies advocating for patient needs, arranging transportation and reminders so that they do not miss important appointments, collaborating and communicating with PCPs and specialists, or simply reaching out to patients to have a conversation on how they are feeling, Kayla is the epitome of what a nurse should be. She is never inpatient and always compassionate when speaking with patients and their families and Kayla sincerely cares. In this day and age of health care burnout, she still possesses the fire and passion to truly make a difference in the lives of our patients. For many of the patients, she is more than a nurse, but becomes a trusted member of their family. I can give you many stories of the positive impact Kayla has had for our patients, as well as myself as a primary care provider. Having Kayla as part of the care team has helped me take better care of my patients with multiple chronic health problems by keeping in close contact with them and communicating concerns, often helping to avoid hospitalizations and worse health outcomes.
