A patient who regularly comes to the clinic came in to see his provider, who was new to the clinic, as well as her nurse. While working up the patient, she noticed as she asked questions that he appeared to be down in spirit. She dug a little deeper, and he opened up to her that he had been depressed for quite some time and had thoughts of taking his life. She immediately reached out to the provider and the psychologist on staff to get this patient some help. The provider did a warm handoff with the psychologist, and the patient was able to get the help that he needed. This was a good catch because she made the patient feel comfortable enough to open up to her after having several visits to the clinic prior to that.

Another situation when a patient who came for a follow-up from a stabbing assault was not able to get a ride and had to have a friend drive him around. She got him signed up for mail order so that meds can be delivered to him. She also reached out to the psychologist again for therapy and the social worker to help him with resources. She has so much compassion for her patients and does not hesitate to assist them in getting the care that they need through collaboration with other team members. She exhibits our organization's ICARE values through compassion, collaboration, respect, and equity.
