Sitting next to her and seeing the whole event unfold, I was so impressed by her calm, caring compassion.
As a Heart Failure Nurse Navigator, Leslie meets with patients during their hospitalization for assessment and to provide education. After a patient is discharged, she makes weekly calls for one month to see if they have any questions or concerns, and to see if they are receiving follow-up from other healthcare providers. Leslie had a 92-year-old patient who lived alone on a remote property. He had two dogs, but his only relative lived out of state. He did not have any other support system.
Leslie carefully followed this patient, calling him, reviewing his medications, discussing any symptoms, following up with his PCP about concerns, etc. She was concerned about him, so she checked on him more frequently than once a week. This week, Leslie became more concerned as he did not answer her calls, nor did he return calls. She also noticed in his chart that he had missed an important follow-up appointment. She reached out to his emergency contact, who also had not been able to reach the patient.
Leslie gathered all the information needed, including details about how many cars should be in his driveway, his dogs’ names, etc. She then called the Josephine County Sheriff’s Department and asked for a well-check visit. She gave them all the pertinent information, as well as her call-back number and the emergency contact information for the patient. A Deputy later phoned back. They visited the patient and found that he was down, unable to get up. His dogs were hungry and thirsty. They did not know how long he had been down, and the patient was very confused. He was transported to ATRMC and admitted to the CCU.
The Deputy praised Leslie for her follow-up. Sitting next to her and seeing the whole event unfold, I was so impressed by her calm, caring compassion. Her nursing instincts were spot on, and she had great follow-through. As a result, this patient received the care he needed. The caring family immediately drove here, grateful to have been informed that he did not suffer tragedy. I am so proud to be Leslie’s teammate, and Asante is blessed to have this wonderful nurse on staff at ATRMC. Thank you!
