Thank you, Jayme, for your hard work and dedication to your patients and their families.
My Uncle was a patient here for several weeks. During his stay, we were informed that he had cancer, but due to his kidneys failing, we were unable to find out if we had treatment options because he was not stable enough to complete further testing or surgery and needed to be transferred to a higher level of care.

Jayme worked hard to try to get a facility to accept him, but due to him not being stable enough for transport, we were not having luck in getting an accepting facility. Jayme was very open and kept us up to date with all of the information she had. As you can imagine, my uncle was in extreme pain and very sick, and one night, while my cousin was sleeping here with my uncle, my uncle decided he no longer wanted to fight; he didn’t want to be transferred. He wanted to go home. My cousin called me to ask to speak with care coordination to see what our options were, and I asked Jayme to call my cousin to answer his questions.

Jayme called my cousin and walked him through all of our options. We knew we could not take him home, as my uncle was a very tall, big guy, and there was no way we could care for him by ourselves. We explained how badly my uncle wanted to go home and how that was not possible, so we asked if we could find a facility that could take him and would be comforting and give him a home-like feeling. Jayme got us referred over to Hoffman Hospice, where we would be able to have a large room that could fit all of his family and friends, and it was set up just like a bedroom and didn’t feel like a hospital or healthcare facility.

Jayme went over the insurance information, and his insurance would only cover a few days once he was stable, and the rest of the time would have to be paid out of pocket. Jayme took the extra step to find another facility that would accept my uncle if his days at Hoffman Hospice ran out. She made sure that all of the paperwork was sent over “just in case” and we had a contact person at that 2nd facility. We appreciate her taking all of those extra steps to make sure that we had a backup plan in place if we needed it. She made sure that we did not have to worry about anything except spending time with my uncle. I truly appreciate her willingness to answer our calls at all hours of the day, to answer any questions or concerns we had, and she also checked in with my family the day after we were discharged to make sure we were doing ok.

We cannot thank her enough for going above and beyond and making sure that my uncle had a comfortable place to peacefully transition to heaven with all of his family by his side. She took so much stress off of his wife and his son, so they could just spend time with him and not worry about the logistics of my uncle being cared for. My uncle's sickness came on so fast, and he declined so quickly that none of us had time to prepare ourselves to lose him, and Jayme made sure that we had all of the answers we needed and took so much stress off our shoulders.

Thank you, Jayme, for your hard work and dedication to your patients and their families. My family appreciates everything you did, and we can never thank you enough for the compassion and empathy that you showed us during one of the hardest times of our lives. We hope that she knows the difference she makes in the lives of not only patients but also their families.
