Beyond honoring a hospice patient's wishes to be at home and avoid prolonged hospital stays, Jennylyn's involvement in this case resulted in a significantly reduced hospital length of stay, thereby increasing access for other patients requiring hospital care.
I am a hospice physician in Yakima. Recently, I had a patient who was on home hospice who had a complication with his chest port, resulting in leakage of his PCA, which severely affected his pain control. The decision was made to transport the patient to MultiCare Yakima Memorial Hospital's Emergency Department to see if IV therapy could fix the port access and the patient could return home. Ultimately, the patient had to have a peripheral IV line placed for the PCA and was admitted to the hospital for further management of the port due to the extent of the complication. Early the next morning, Jennylyn assessed the patient's site and promptly messaged me. In addition to her excellent report, Jennylyn showed a clear understanding of the patient's situation (on hospice and wanting to return home as soon as he could) and indicated that she was going to try and restore his port access as soon as safely possible.
Within just a few hours, Jennylyn had restored the port's patency and had reconnected the PCA. He was monitored for a bit longer to ensure the line remained patent and that he would be safe to discharge. Throughout the process, Jennylyn maintained excellent communication with me and the patient's hospital team. The patient has been at home now for >24 hours, and everything has gone very well. He has expressed significant appreciation for IV therapy's ability to restore the port access promptly so he could return home.
Beyond honoring a hospice patient's wishes to be at home and avoid prolonged hospital stays, Jennylyn's involvement in this case resulted in a significantly reduced hospital length of stay, thereby increasing access for other patients requiring hospital care. Beyond my personal gratitude, the patient, the hospice team, and the hospital nurse have all expressed their appreciation for what she was able to do in this case in order to reach the best possible outcome.
Though she shows it every day, the care provided by Jennylyn in this specific case truly emphasizes our shared values, particularly:
-Integrity: Going above and beyond to do the right thing for this patient and in line with his goals of care.
-Stewardship: Increasing access to resources for all patients by reducing the time of hospital admission for this patient.
-Excellence: Communicating clearly and showing respect while delivering results.
-Collaboration: Taking initiative to contact the physician and have additional conversations with the care team.
-Kindness: Listening to all stakeholders (patient, RN, physician) and responding in a way that maintained clear goals and expectations.
