I am honored to nominate the exceptional 4 East nursing team for the DAISY Team Award in recognition of their unwavering commitment to patient safety, clinical excellence, and compassionate, relationship-based care. This dedicated team consistently upholds the highest standards of nursing, as evidenced by their remarkable achievements in zero harm initiatives. Their last CLABSI occurred in August 2018, and their last HAPI in June 2023—a testament to their vigilance, diligence, and commitment to infection prevention.
Beyond these impressive clinical outcomes, what truly sets the 4 East team apart is their ability to translate compassion and empathy into meaningful action, profoundly impacting the lives of their patients. A powerful example of their exceptional care is their work with a young man in his 40s with developmental delays who spent nearly a year at The Miriam Hospital, including 5 ½ months on 4 East.
When this patient first arrived at The Miriam, he was ambulatory; however, upon transfer to 4 East, his mobility had significantly declined, and he was bedbound. Recognizing the profound impact this had on his overall well-being, the nurses and CNAs worked together daily, ensuring he had the support and structure he needed to thrive. The team developed a personalized behavioral plan, carefully designed through the lens of a caring, empathetic provider who understood that this patient should be walking again. With unwavering dedication and encouragement, the team worked with him every single day, slowly rebuilding his strength and confidence. Through their consistent efforts, he progressed from being completely bedbound to ambulating daily.
This remarkable transformation was not just about restoring physical mobility—it was about enhancing his quality of life and behavioral stability, demonstrating the 4 East team’s commitment to holistic, patient-centered care. After almost a year at The Miriam, he was successfully placed in an appropriate setting, thanks in large part to the extraordinary efforts of this team.
If ever there was a living example of Joanne Duffy’s Quality-Caring Model, it was in the care provided to this patient. Their actions exemplified the professional practice model and caring factors that define nursing at The Miriam Hospital—true relationship-based care at its finest.
It is with immense pride and admiration that I nominate the 4 East nursing team for the DAISY Team Award. Their dedication, passion, and extraordinary contributions to patient care make them truly deserving of this prestigious recognition.

The DAISY Team Award is a prestigious recognition that honors exceptional collaboration and the collective efforts of nursing teams dedicated to extraordinary patient care. 4East exemplifies these qualities through unparalleled teamwork, unwavering dedication, and profound compassion. Our nursing staff works in harmony to create an environment where patients feel safe, valued, and well cared for. The synergy within our team allows us to provide the highest standard of care, ensuring that every patient receives the attention and support they deserve.
First and foremost, the nurses on our unit exemplify exceptional teamwork. From the moment a patient enters our unit, our team comes together to provide seamless care that addresses every aspect of the patient’s needs. Whether it is coordinating care with doctors, physical therapists, pharmacists or other healthcare professionals, our team communicates effectively to ensure that each patient receives the most comprehensive care possible. This collaborative effort allows us to address complex medical issues from multiple angles, ensuring that no aspect of a patient’s well-being is overlooked.
Our team also consistently demonstrates the power of mutual support. In high-stress situations, such as handling emergencies or managing patients with complex conditions, our nurses and staff step up for one another, providing both professional and emotional support. There is a deep sense of camaraderie on our unit, where every team member is valued and encouraged to contribute their expertise. Whether it’s stepping in to assist a co-worker in need, offering a word of encouragement during a difficult shift, or helping to manage the unit’s workflow, each person plays a vital role in creating a positive, productive, and patient-centered atmosphere.
Our team is consistently dedicated to improving patient outcomes. One of the standout qualities of our unit is our ability to adapt to challenges while upholding the highest standards of care. A prime example of this was when we cared for a patient who remained on our unit for five and a half months, presenting daily challenges. To protect both the patient’s well-being and our staff from burnout, we rotated team members daily while ensuring exceptional care. Through continuous support and encouragement, we successfully helped the patient regain the ability to walk, achieving remarkable progress each day.
Our commitment to patient safety, evidence-based practices, and continuous improvement is evident in everything we do. By working together and learning from each experience, we can create a culture of excellence that benefits every patient who walks through our doors.
Whether responding to unexpected surges in patient volume, navigating new healthcare protocols, or adjusting to changes in patient needs, our team is flexible and resilient. We rise to meet every challenge with a calm, collective approach, ensuring that our patients receive the best possible care, even in the face of uncertainty. This adaptability is a testament to the strength and professionalism of our team.
In conclusion, 4East deserves the DAISY Team Award because of the outstanding teamwork, dedication, compassion, and patient-centered care demonstrated by our nursing staff. Through our seamless collaboration, mutual support, and unwavering commitment to our patients, we create an environment where excellence in nursing care is the standard. Our ability to adapt to challenges, mentor new staff, and advocate for our patients further solidifies our deservingness of this prestigious recognition. The DAISY Team Award would be a well-deserved acknowledgment of the collective efforts of our unit in delivering health with care.
