I am writing to nominate Jess Troyer, an exceptional nursing student with extraordinary clinical skill and compassionate care, for the prestigious DAISY Award. This nomination reflects Jess’s unwavering commitment to patient care and her ability to navigate complex healthcare systems to ensure timely and effective treatment for those in her charge.
Recently, Jess was pivotal in addressing a challenging situation involving a patient’s discharge delay due to the need for a Zio Patch and the absence of a Primary Care Provider (PCP). Jess’s proactive approach in messaging post-acute planners for an urgent PCP referral and her insightful contributions to the discussion on the intricacies of PCP availability for women of childbearing age were instrumental in resolving the issue. Her suggestion to the bedside nurse and unit manager to consider the OB provider as an interim PCP to receive the Zio Patch report showcased her innovative thinking.
This initiative prevented a discharge delay and facilitated seamless coordination of system resources, ensuring the patient received quality care and follow-up. Moreover, Jess’s engagement in addressing the barriers faced by the patient, who had not received care until the third trimester and lacked documented pulses before delivery, highlights her dedication to comprehensive patient care. Her ability to foster robust discussions among healthcare professionals and her advocacy for the patient’s need to establish care with a PCP are commendable.
