Even when busy, she prioritizes patient care, ensuring they receive the best possible support and resources.
Amber has been in the Ambulatory Care Manager role for just over one year. She has excelled in that time and enrolled dozens of patients in chronic care management so she can assist the PCP with following these complex patients more closely and providing the best care for them.
Amber is always conferencing with other members of the care team, including MSW and PharmD, to determine the best plan and resources for her patients. She is truly dedicated to her patients and her practice.
