I am pleased to nominate Emily Bates for her immediate impact. She leads with a clear mind and full heart each day, serving as a fierce advocate for her clients. She has managed multiple complex situations with poise. Recently, Emily performed a case open for a client who was in great need. The client's mother recently passed away, leaving the client in the care of another family member who was unfamiliar with their care.

Emily identified both a lack of knowledge and a lack of supplies in the home. Prior to leaving the home, she helped the new caregiver identify necessary resources for the patient's well-being. Emily immediately stepped in to set up medical equipment and supplies for the patient, as they had none in the home, and assisted in setting up respiratory therapy to come for in-home guidance and education. She spoke with the physician's office to set up necessary follow-up and educational sessions. She assisted in identifying the physicians and obtaining medical records to assist the family member. She also strongly advocated for staffing this client prior to obtaining an authorization, despite the cost, which Centria then did as it was the right thing to do for the health and safety of the patient.

Without the initial assessment and deep dive on the ability level of the caregiver, the client and family would not be in the spot they are in today. Emily approaches each day with a can-do attitude. She has risen to every occasion and is quick to shift her priorities based on the needs of those she serves. She is very detail-oriented, which is to the benefit of our patients.

She completes meticulous reviews of her patients' care plans, adding critical information. She identifies risks and promptly acts, often working early mornings and late nights to ensure the best possible outcomes. Beyond the clinical knowledge and skill, Emily is a great member of our team. I thoroughly appreciate her positivity on tough days, a great example of how a change in outlook can reshape any challenge. She has a growth-oriented mindset that continues to pay dividends for our patients, their families, and our entire healthcare team. Thank you, Emily, for all you do and for who you are.
