During one recent encounter, Amanda and I spoke with a patient who was preparing to be discharged to a SNF after a 4-month inpatient hospitalization. Amanda encouraged the patient about how he was going to do fine. She offered to help him shave so he looked good. She reminded him of how well he had done and commended him on his positive actions. She reminded him of his abilities and encouraged him as he prepared to leave the next day.
