We are proud to recognize a PCC who went above and beyond to support a patient with complex needs. The patient and their family were facing significant financial challenges and lacked the resources needed for admission to a skilled nursing facility (SNF). Despite multiple efforts by the hospital team to complete the Medi-Cal referral, including the patient’s son visiting the Medi-Cal office, progress stalled.

Upon learning about these struggles, Prajwala Narla took the initiative to identify the root cause of the issue. She dedicated time to sit down with the patient and family, walking them through the referral process in detail. Prajwala explained each step clearly, ensuring they understood how to complete the application successfully. Praj’s compassionate approach made a tremendous difference.

The patient’s son, who was also dealing with his own challenges after a recent accident, expressed deep gratitude for her support. Patient was successfully transitioned to the next level of care.
