Johanna was the Nurse Navigator for an elderly patient who was bedded on 5M at Chandler. The patient had multiple co-morbidities and limited capacity, and his wife was his primary caregiver at home. The care team was made aware the patient's wife had suffered and unexpected emergent condition and became a neuro ICU patient here at UKHC, and her condition was not expected to have a positive outcome.

After finding out about the situation, Johanna offered to take the husband (bedded on 5M) over to the ICU to visit and spend time with his wife. Due to his health conditions and inability to be left alone, Johanna spent several hours sitting with the couple, allowing the patient's husband to spend extra time with his wife. This type of patient care is not in Johanna's job description nor an expectation of her role. Understanding the severity of the situation, and the limited time the wife had, Johanna took it upon herself to ensure this husband and wife were able to spend some precious time together.

Ultimately, the wife passed away, and although the husband wasn't fully aware of what was going on, he did understand that his wife was sick and was grateful to be able to spend time with her. This is a true example of what a DAISY nurse is, and Johanna went above and beyond to ensure this patient and his wife were able to spend time together at the end of her life.
