Ms. Hunt consistently provides this exceptional level of care to her patients. Her knowledge of clinical workflows exceeds all expectations of a case manager.
Ms. Hunt provides follow-up care for high-risk, non-enrolled discharged patients. She has taken this role to new professional and clinical heights by ensuring that all discharge needs are completed and clearly understood by both the patient and the patient’s support system at home.
Ms. Hunt spends significant time reviewing clinical documentation to understand each patient’s complete medical history, medication profile, and pending specialty referrals. An example of her dedication, clinical expertise, and empathy is demonstrated in the following patient care encounter.
During a discharge follow-up call with a high-risk patient, Ms. Hunt identified that the patient had been discharged with incorrect medications. She immediately advised the patient not to take any further doses and provided clear instructions to return to the Emergency Department or call 911 if necessary. Ms. Hunt assured the patient that she would contact the discharging provider, resolve the issue, and call back as soon as possible.
She promptly contacted the discharging provider, informed them of the error, updated them on the patient’s current condition, and requested immediate correction. While the providers were correcting the medication error, Ms. Hunt and the discharging provider contacted the patient together to ensure that she had stopped taking the medication and properly discarded the remaining doses.
On a daily basis, Ms. Hunt also assists patients with referrals that have been incorrectly placed or not placed at all. She works closely with attending providers to correct errors, contacts Humana or the VA to assist with specialty assignments, sends records to specialty providers, and helps schedule appointments.
Ms. Hunt always provides patients with her contact information and, even though she is not their assigned case manager, continues to support them well after discharge. She is also known for personally walking to the pharmacy to pick up medications and meeting patients at the Rock Merritt front doors. She has done the same with durable medical equipment and personal items that were left behind after discharge.
These actions not only make the transition home easier for patients but also contribute to meaningful service recovery for the organization.
