The ¡Salud! Services team exemplifies team excellence through their unwavering compassion, seamless collaboration, and deep commitment to ensuring every patient receives the care they deserve.
The ¡Salud! Services healthcare model is designed to reduce barriers to care for vineyard workers and their families by integrating clinical care with outreach, care coordination, and social support services. This nurse-led team, guided by Clinical Nurse Manager Ericka and supported by nurses Jessica and Cris, program assistant Janely, and administrative outreach coordinator Claudia, works holistically to address both medical and social determinants of health.
In 2025 alone, the team supported 1,808 registered patients ranging in age from 14 to 90, representing 12 different languages. Their effectiveness is rooted in constant, real-time communication with one another to move patients efficiently through the care continuum. This coordination takes shape through frequent phone calls, texts with patients, and in-person outreach, ensuring needs are identified quickly and addressed thoughtfully. Whether organizing care plans, navigating complex systems, or conducting home visits, they operate as a highly coordinated unit with a shared purpose: improving health outcomes through trust, access, and continuity of care.

During the winter SNAP funding freeze, Janely and Cris personally coordinated and delivered boxes of food to patients in need, ensuring families had access to basic nutrition during a critical time without having to leave their homes.
In another instance, Jessica supported a patient who experienced a serious eye injury that threatened both their health and ability to work. She not only acted quickly to arrange urgent and follow-up medical care, but also helped secure rent assistance for six months during their recovery. This comprehensive, compassionate response ensured the patient could focus on healing without the added stress of housing insecurity.
These moments reflect the team’s ability to combine clinical expertise with creativity and empathy to solve complex challenges. Each member brings unique strengths, yet together they create a unified, patient-centered approach rooted in integrity and respect.
