After a patient was discharged and declined services, she reached out to better understand his concerns. He shared that he refused to let anyone into his home due to a severe roach infestation and unsafe conditions, explaining that previous service providers had closed his case because the home was not safe to enter.

Rather than accepting this barrier, Natalie actively listened and uncovered that the patient and his significant other needed wrap-around community support, including help cleaning and stabilizing their home.

She connected them with a community social worker, who visited the residence and observed holes in the flooring and significant infestation. The social worker was able to arrange assistance through a faith-based pest control company.

Before services could begin, the patient was readmitted. Natalie immediately visited him and had a heartfelt conversation about transitioning to an SNF for a safer plan.

Because of the trust she built, the patient opened up about his personal journey and agreed not only to the SNF placement but also to allow Natalie to contact DCF to provide additional support for him and his significant other.

Natalie's actions reflect deep compassion, persistence, and an unwavering commitment to safety and dignity.
