Ana was the nurse care manager for an extremely medically and socially complex veteran. He recently passed away earlier this week after several months of increased health issues and care needs. She has been exceptional in her care of this man and I know that she truly made a difference in his life and helped him to have a dignified and peaceful death.

Regarding the latter, his largest and most recent goal was to die at home. Her care directly resulted in assistance to be able to do so. Although my heart is heavy with his passing, it is made lighter knowing that the HBPC team -specifically Ana- made a huge difference in his life. On the morning of his passing, his son called to let me know before he told many family members and hospice. He made specific mention that some of the Veteran’s final expressed thoughts were to make sure that his son communicated his gratitude for Ana. It is a great privilege to care for Veterans in their home and walk with them through the end of their journey. I can think of no higher compliment than what was expressed by this Veteran. Ana has been an amazing colleague to partner with. I feel very lucky to have been able to work with her to care for this veteran and be able to continue to work with her in service of others.
