Candace is a former ICHD RN who has been working as a PD RN for the last 3 years. Candace has maintained the census at her home facility, Brenham, while assisting at numerous other facilities up to 50 miles away from her home facility. While working as a PD RN she has helped many patients, including helping a patient through the end of their life. Mr. R was a PD patient that was fairly immobile and needed assistance coming into the clinic. Candace made a point to always assist the patient in and out of their vehicle, and blocked off extra time to ensure this patient and his wife had time to ask questions and feel confident in their treatment. Mr. R contracted COVID in early 2021, after traveling, and Candace supported this patient and his wife. She ensured he partnered with his nephrologist to remove supplements that were harming him, and continued to support him as he approached the end of his life. She spoke with his wife almost daily to ensure they were supported and felt cared for. Ultimately, Candace ensured Mr. R retained his dignity at the end of his life and his wife was prepared for next steps. Candace recently took on the role of Clinical Coordinator at Brenham and is now supporting home and ICHD in achieving superior clinical outcomes. She is a true DAISY Nurse.
