Catherine collaborated with all members of my family to ensure that we understood my dad's plan of care and were in agreement of the best course of action. She wasn't in a rush and listened to ongoing concerns related to the care of my father. She spoke with my brother on the phone for over an hour explaining Dad's situation and also took time to talk with him in person. She advocated for homecare in the discharge planning to ensure we would have the support we needed. She truly listened to us and helped us to understand and prepare for his discharge. She is a great patient advocate; she shows compassion and caring through her actions.
