I am delighted to nominate Haley Kaufman for The DAISY Award, recognizing her exceptional contributions in patient care and management within PCMH.

Patient Care Excellence: Haley exemplifies the highest standards of clinical care. Her diagnostic expertise and passionate approach have transformed patient outcomes and experiences as a Quality and Population Health Nurse. Her dedication to patient-centered care is reflected in her thoughtful communication with patients and their families, ensuring they are well-informed and supported throughout their treatment journeys. Haley has been involved in patient care in other areas as well, including Occupational Medicine, where she has organized and attended health fairs for community businesses.

Mentorship: Haley is a recognized educator and mentor, deeply committed to helping all employees. Her dynamic teaching methods and hands-on mentorship have significantly impacted the development of both new and seasoned staff members. She has been instrumental in designing and leading training programs, contributing to onboarding, and providing invaluable guidance that fosters both professional and personal growth among her trainees. She is definitely the clinical “go to” person. Haley jumps in to help anywhere she is needed, no questions asked, from covering the front desk, to clinical help and even going above and beyond assisting Dr. Syler when he needs physical assistance.

Management and Leadership: Maybe most importantly as to why I am nominating Haley, in addition to her clinical and quality roles, Haley has significantly contributed to the management and operational excellence of our department. Haley took the interim Practice Manager role after the previous Director left and continues to act in a Clinic Leadership role. Haley started her career at PCMH as a Front Desk Receptionist, while taking classes to become a Certified Medical Assistant. Once that training was completed, she then went back for more education to become a Registered Nurse. She held the role of Staff RN until she took the position of Quality Nurse, because she was yet again continuing her education to become a Psychiatric Mental Health Nurse Practitioner, where she graduated this past May. Haley has been instrumental in assisting with changing the culture of Tell City Clinic, where the turnover rate was terrible. Together, we have changed the workflow and dynamics between staff members. Haley will be transitioning into her Nurse Practitioner role within the next few months, and she will be deeply missed, although I am excited to see how she expands and enhances our Behavioral Health services. It is with great enthusiasm that I submit this nomination, confident that Haley’s outstanding achievements make her a deserving candidate for The DAISY Award.
