Jackie contributes to the delivery of quality compassionate patient care by ensuring each patient and family member gets involved in their care. She provides detailed instructions on their diagnosis and medications. She has embraced her new role as transition nurse and has contributed to very positive outcomes, decreased readmissions and improved HCCAP scores. Jackie participates in the daily interdisciplinary huddles and adds valuable information about the patient's level of understanding and the family's involvement in their care. She is an advocate for the patient. Jackie demonstrates compassion by spending time to get to know the patient and family and she does follow-up phone calls after discharge and even continues to call the patients who are at greatest risk for failure at home. She feels a personal responsibility to each person she is privileged to serve.
