A patient and his wife came to a nursing unit asking for his wound dressing change. Unit nurses couldn’t understand why he came back for a dressing change after he was discharged home with home health the day before. They found out NP Kathy Luo was to follow this patient and asked for help. Kathy took the time to speak with the patient and the wife using the interpreter service and reviewed the chart to find out the home health was arranged to the address that was in Facesheet, not to the address this patient was actually residing in. This patient ended up not getting the home health in time, and Kathy got to intervene. Kathy coordinated with the nursing unit manager and get the dressing changed immediately and worked with care coordinator to correct the home health arrangement. The patient and the wife left reassured to receive the appropriate care from home health in the correct address. Thank you, Kathy, for your patient and thorough work to help our patients.
