Linda had a VERY difficult discharge on Med/Surg.
Linda persisted with concerns about the patient's transportation home and worked with the physician, clinical supervisor and house supervisor to provide a different way to get the patient home more safely.
Linda advocated for another patient who wanted to be on comfort care and coordinated with the family, physician, and case management to come up with a plan. She worked quickly and efficiently with compassion to see that the patient's wishes were attended to.
