Lisa has served with Children's at the bedside for 20 years and in the position of Case Manager for the last 10 years. Throughout her career, she has advocated for her patient's emotional and physical needs and displayed moral agency in a variety of ways.
Being a Case Manager requires the daily assessment of a family's medical and social needs. Lisa initiates with families on a personal level to find out exactly what a patient's family requires to succeed upon discharge home. This can range from involving Social Work to provide parking and meal assistance to securing formula and needed equipment, despite all barriers. She advocated for them by spending hours talking with insurance agencies and home health companies helping families obtain coverage. In order to help streamline this process, she served as a valued member of the Continuity of Care Committee and her insights into processes have been implemented throughout Case Management.
She is meticulous in her documentation and seeks to understand the unique, changing needs of patients and families, truly embodying moral agency. Even after the discharge process is complete, Lisa follows up with families to ensure that their questions are answered, and their needs met. This single act has prevented patient readmission for preventable causes. Providers trust her judgment and seek her out for her insight and experience and her supervisors trust that any tasks she is assigned will be completed quickly and accurately.
One of Lisa's colleagues recounts, "I truly appreciate all she does through the hospital admission, discharge and beyond. I recently had several complex patients who needed to be discharged and one required a new DME company. Fortunately, she had been proactively working with the new company and these complex patients were discharged smoothly and efficiently."
Lisa advocates not only for patients and their families but for team members. She is an excellent mentor and teacher and is sought after by orientees. She advocates for staff members through encouragement and guidance, often suggesting learning opportunities and resources. Additionally, she served on the Sunshine Committee to foster a positive work environment.
On a personal level, she is seen working tirelessly, well beyond her scheduled 40 hours a week, to make sure that no child or parent is left without the support they need while in the hospital, as well as at home following discharge. She does all this with a cheerful attitude and willing heart. She regularly works 10 hours at the hospital, goes home and bakes treats for the staff and comes back to work the next morning.
Lisa's experience, knowledge, and dedication to her patients, families, and coworkers are truly outstanding and represents what Children's Health stand for.
