From the moment of transfer until discharge, through readmission and ongoing communication post-discharge, Louise has advocated and supported this client’s safe, evidence-based care.
Louise not only delivers exceptional client and family-centred care but also provides valuable leadership and collaboration to the SODR team. Louise is committed to life-long learning, as evidenced by her diabetes educator certification and skill set as well as leadership in supporting prescribing for Methadone. Louise demonstrates a commitment to evidence-based and holistic client care with her work with our SCI patient population, an area where she has built trust and expertise over the past few years. I have had the pleasure of working with her on a bowel management standard of care as well as seeing her leadership skills when she brought the outpatient spina bifida together with the inpatient nursing team in order to build a stronger pathway for these clients. I could speak to her clinical skills by multiple examples. However, one unique client’s story stands out. Louise was involved with a young girl on SODR who was transferred to us about a year ago. She had an acute event that left her with a spinal cord injury. From the moment of transfer until discharge, through readmission and ongoing communication post-discharge, Louise has advocated and supported this client’s safe, evidence-based care. The family dynamics were complex, but Louise continued to invest in the therapeutic relationship and went above and beyond to ensure this client was safe for discharge. I remember our collaborative medication education sessions. This client’s success is directly correlated to Louise’s tireless efforts to provide education, support, and a safe transition. It is my pleasure to nominate Louise for this award. She truly exemplifies the vision, mission, and values of HB. She is an inspiration to work with and our clients/families are lucky to have her level of dedication as a part of their care.
