A Veteran was assaulted in a Walmart parking lot. He reached out to the PACT team via secure messaging, and the patient was advised to go to the local ER to be evaluated for concussion/Traumatic Brain Injury. As the  PACT team was unable to reach the Veteran via phone, they requested a welfare check for the Veteran. The Veteran was then reached and followed instructions to go to the local ER for further evaluation.

A few days later, the PACT team received an update from Community Care. The Veteran underwent craniotomy with evacuation of a subdural hematoma. Post-operatively, he will be discharged to a Skilled Nursing Facility for a short stay then home, possibly with home health. The prompt action of the PACT team and their coordination saved the Veteran’s life.
