Recently, it was brought to our attention that a patient had not gone to primary group, which was unlikely. Nursing did rounds and found the patient to be in his room, staring out a window, looking very disconnected, and had a note/will written to his wife. Nurse Nicole immediately started her assessment. She asked all the right questions and engaged with the patient with compassion and urgency. The patient was suicidal, with a plan and means to carry it out. The patient shared his plan with her and reported, "If you came in any later, you would have found me in the bathroom in a pool of blood." She stayed with the patient, offered him support, notified the appropriate parties (psych notified, admin on call notified), and was able to safely transfer the patient to the hospital. The patient has since been cleared to return back to RCA and, per notes, motivated for continued treatment. She continuously goes above and beyond for every patient and supports her team. She is an example of what a nurse should be.
