Sara noticed that a patient who was admitted was the primary caregiver for her husband at home. The only available family lived in Canada and were unable to come down secondary to border restrictions. Sara contacted the patient's husband's primary care provider and set up Home Health immediately so that the husband had support and eyes on him at home. Unexpectedly, our patient passed away in the hospital and the home health social worker was able to immediately present to the patient's husband's home to help with coping support and planning. Sara's actions went above and beyond and really brought some light to a sad and unexpected situation for our patient's husband.
