An elderly patient was brought into hospital with breathlessness, confusion, and a general decline in her health. Her family recognised that she was deteriorating and coming to the end of her life. Sarah supported the family, listening to their distress and concerns about the situation and their desire for their loved one to have a peaceful and dignified death.

Sarah advocated above and beyond to support the family, including referring to the Palliative Care and Chaplaincy teams. Through Sarah's actions and advocacy, the family was able to share the last few hours with their loved one in a peaceful and dignified manner that they all deserved.
