Sarah cared for a patient being transitioned to hospice who had a very large family. Sarah worked all shift to help transition the patient, work with hospice and the family to make him more comfortable. She spent a great deal of time explaining and educating the family. As I was leaving at the end of my shift, the patient's daughter was sitting on the floor outside the room and Sarah was down on the floor with her, providing comfort and explaining again about hospice and how they could make her dad comfortable. Sarah spent a great deal of time with the family helping them through such a difficult time.
